Simple Protocol

Know what to expect when you visit Florijuana Medical Center for the first time. Please read the protocol below to familiarize yourself with our process and print off the information here to keep as a reference.

Information We Need

After scheduling the first consultation, the prospective patient must send us the following information via fax. You can fax us at 561-330-0724.

  • Full name
  • Date of birth
  • Contact information
  • Current address
  • Proof of Florida residency (driver's license number, etc.)
  • Parents' information in case of minors
  • Recent (within 2 years) medical records documenting a diagnosis, testing, and treatment of your qualifying medical condition. Documentation from a specialist is best, but can also come from your primary physician.
  • Initial visit $250
  • Follow-ups $150
  • At this time we do not accept any insurance. We accept cash and all major credit cards.

Next Steps

The doctor will review your information, and we will call you back as soon as possible to schedule your appointment. Not all patients qualify for medical marijuana at this time. Florida law is subject to change. We expect different medical conditions to be added in the future and will keep you posted on the conditions that qualify for medical marijuana treatment.

Remember that Florida law mandates that you must be our patient for at least three months before you are allowed to purchase medical marijuana. During this three-month period, you may need to be seen again for regular follow-ups. Our doctor may order any testing that needs updating.

The doctor will add you to the Florida registry according to the Florida Compassionate Care Use Act. You will then be issued and receive a Florida ID card. The doctor will then write a customized pharmaceutical order for a maximum 45-day supply. We do not accept insurance at this time.
